Terms of Service

Introduction
By using Bloom Concierge MD’s in-person or telehealth psychiatric services, you agree to these Terms of Service (“Terms”). These Terms are designed to ensure clear expectations, professional boundaries, and patient safety.
Services
We provide psychiatric evaluations, medication management, and therapy services for adults, parents, and young adults via in-person and telehealth appointments. Additionally, we aim to deliver care that is consistent, compassionate, and effective.
Patient Responsibilities
Moreover, you must provide accurate health information, attend appointments on time, use private environments during telehealth, and communicate respectfully. Furthermore, adherence to these responsibilities supports successful treatment outcomes.
Telehealth Consent
Clinicians conduct telepsychiatry sessions using HIPAA-compliant platforms. However, participation requires your agreement to engage from a private space. In addition, we offer services to residents of South Carolina and Maryland.
Confidentiality
We protect all information in compliance with HIPAA. Nevertheless, exceptions occur only when safety or legal obligations require disclosure.
Fees and Payments
You must complete payment or insurance verification before receiving service. Also, administrative fees may apply for missed appointments or non-clinical documentation.
Cancellation and Missed Appointments
Cancellations require 24 hours’ notice. Otherwise, missed or late cancellations may result in fees or termination of care after repeated occurrences.
Emergency Care
We do not provide emergency or on-call services. Therefore, for urgent crises, call 911 or the Suicide and Crisis Lifeline at 988.
Communication Policy
We cannot answer clinical questions via unsecured email or text. Instead, secure messaging occurs through HIPAA-compliant platforms only.
Professional Boundaries
Dual relationships and personal contact outside of clinical settings are prohibited in order to protect professionalism and care quality.
Termination of Care
Either party may end the therapeutic relationship. When this occurs, appropriate referrals will be offered to ensure continuity of care.
Limitation of Liability
While we aim to provide excellent care, Bloom Concierge MD cannot guarantee specific outcomes. Ultimately, liability is limited to the cost of services rendered.
